Last chance to
enroll in new HFM BOCES health plan
Final enrollment/help session, 2:30 - 5 p.m. on Thursday, June
14 in HFM BOCES Conference Center (Distance Learning Room C103)
12, 2012 – It is necessary for all employees and retirees
to enroll in a new health insurance plan by Thursday, June 14.
(This is an extension beyond the June 11 deadline.)
Do not allow
your health coverage to lapse. The current HFM BOCES
health insurance plans expire on June 30, 2012. The new health
insurance plans become effective on July 1, 2012.
If you have already
completed your enrollment, thank you for your cooperation.
CDPHP, Bouchey & Clarke Benefits and NBT Bank will be on hand
from 2:30 – 5 p.m. on Thursday, June14, in the HFM BOCES
Conference Center (Distance Learning Room C103) to help
employees and retirees enroll in the plan of their choice.
This final session
provides an opportunity to have any questions answered, and
allows the enrollment process to be completed in a single step.
The NBT account for those selecting the Health Saving Accounts
option will also be set up at this time.
Enrollment may also
be completed online, but in that case, those selecting the
Health Saving Accounts option still need to establish their
savings account separately with NBT Bank.
Important for those selecting the HSA
account: You must bring a form of ID (driver’s
license) and your Social Security number (SS#), as well as the
SS# and date of birth for anyone (e.g. spouse) you want to have
access to your HSA account.
HAVE TAKEN PAPERWORK HOME FOR NBT BANK MUST EITHER BRING THE
PAPER WORK TO THE ENROLLMENT SESSION ON THURSDAY, JUNE 14 OR
RETURN IT TO AN NBT BANK LOCATION BY JUNE 15.
