necessary for all employees and retirees to enroll in a new
health insurance plan by June 11. If you have already completed
your enrollment, thank you for your cooperation.
Representatives from CDPHP, Bouchey & Clarke Benefits and NBT
Bank will be on hand from noon – 5 p.m. Monday, June11, in the
HFM BOCES Conference Center (Room 112B) to help employees and
retirees enroll in new health insurance plans that become
effective on July 1, 2012.
This session provides an opportunity to have any questions
answered, and allows the enrollment process to be completed in a
single step. The NBT account for those selecting the Health
Saving Accounts option will be set up at this time.
Enrollment may also be completed online, but in that case, those
selecting the Health Saving Accounts option still need to
establish their savings account separately with NBT Bank.
Important for those selecting the HSA account: You must bring a
form of ID (driver’s license) and your Social Security number
(SS#), as well as the SS# and date of birth for anyone (e.g.
spouse) you want to have access to your HSA account.
If you cannot take advantage of online enrollment or Monday’s
scheduled session, a printed enrollment form may be requested in
person from Linda Edwards in the superintendent’s office
(736-4681, ext. 4696). The form must be completed and returned to Linda
Edwards by June 11.
